Medical Appointment Transportation at Community Services Center is here to help you get to your medical appointment on time with excellent customer service. Hours of operation are: Monday-Friday and we can accommodate early or late appointments.

Disclaimer: Community Services Center has a zero tolerance policy regarding drug and alcohol use by transportation service drivers.

How to schedule ride?

Appointment rides have to be made through your insurance carrier. Behind your insurance card is a phone number that must be called and ride set up. All rides must be made 72 hours before the appointment. After that, ask for Community Services Center to be your transportation provider.

What if my ride is multiple times a week?

Do you have dialysis or another type of appointment that you need be at multiple times a week, Medical Appointment Transportation can make sure you make it to all your appointments.

Drivers

All Medical Appointment Transportation staff are CPR/AED/First Aid certified, approved defensive driving and PASS wheelchair licensed. 

What kind of vehicles do we have?

We have 3 wheelchair accessible vans: 2015 Toyota Sienna, 2014 Toyota Sienna, and 2013 Toyota Sienna, a 2015 Nissan Rogue, and 2011 Toyota Tacoma.
